Non-microbial etiology: periapical cysts sustain post-treatment apical periodontitis

INMICROSCOPY, AS INNATURE,ONERECOGNIZESONLYWHATONEALREADYKNOWS. Routine histopathological diagnostic reports and publications based on retrospective reviewing of such, perpetuate the notion that nearly half of all apical periodontitis lesions are cysts. Studies based onmeticulous serial sectioning of apical lesions retrieved in toto show that the actual prevalence of cysts is only about 15% of all apical periodontitis lesions. Periapical cysts exist in two structurally distinct classes, namely those containing cavities completely enclosed in epithelial lining (periapical true cysts) and those containing epithelium-lined cavities that are open to the root canals (periapical pocket cysts). From a clinical point of view, small cysts, particularly the pocket cysts, can heal after non-surgical root canal therapy whereas large cysts, mainly true cysts, are less likely to be resolved without surgical intervention.

Non-microbial etiology: foreign body reaction maintaining post-treatment apical periodontitis
The polymerase chain reaction (PCR) is an elegant technology for faithfully replicating and amplifying the master molecule of life, but a valid scientific procedure remains essential without which the resulting data have only very limited value. The presence of microbial infection in the complex apical root canal system is the major cause of posttreatment apical periodontitis in well-treated te...

Most periapical lesions resolve after root canal treatment of teeth with primary infections. Over the last decade there has been a renewed focus on post-treatment apical periodontitis and its etiology. This review describes the microbiota associated with persistent post-treatment infection, including microbial identification, ecology, and environmental selection. Compared with untreated teeth, ...

Apical periodontitis is an inflammatory disorder of periradicular tissues caused by aetiological agents of endodontic origin. The subject has been reviewed by Nair. (1)(2) They are believed to result from inflammatory processes in the periapical tissues associated with bacterial infection and necrosis of the dental pulp in carious teeth. As a consequence of these processes and the inability of ...

Traditionally, endodontology includes pulp and periapical biology and pathology. Clinically, however, endodontics is perceived as treatment of the root canal with fi les and the placement of a root fi lling, or treatment by surgical endodontics. The vital pulp and the treatment measures to preserve its vitality are usually considered a part of conservative dentistry, and include specifi c techn...

BACKGROUND Apical periodontitis includes periapical granulomas and radicular cysts, which are histologically distinguished by the absence and the presence of an epithelial lining, respectively. The main cause of apical periodontitis is the bacterial colonization of the root canal space. This research aimed at assessing whether and how periapical granulomas and radicular cysts differ in terms of...
